Physicals Needed
Just a reminder…Incoming Kindergarten, Sixth, & Ninth graders need a current physical and immunizations on or before the FIRST day of school to AVOID First Day Exclusion. Incoming Kindergarten students are REQUIRED to provide an eye examination form by October 15, 2010. Students entering Kg. and 2nd grades are now required to provide a DENTAL exam, dated between November 15, 2009 and May 15, 2011 for current school year. A doctor must complete the lead level screening (for Kg) and diabetes screening for all students. Parents: REMEMBER to complete, sign, and date the Health History portion of the physical or it WILL NOT BE ACCEPTED. SIU-E Community Nursing will offer school/sports physicals and immunizations during Registration at CMHS: Thursday July 29 1:00 – 6:00 PM Friday July 30 8:00 am – 12:00 Monday August 2 1:00 – 6:00 PM Cost: Physicals $35.00 Immunizations $6.50 They are taking appointments at 482-6959. You need to bring a copy of your child’s immunization record with you. Accepted forms of payment are cash, check, Medical card/Allkids. Charge cards will not be accepted. |
